Individual Client/Family Public Health Nursing

• Targeted Case Management to a specific populations (i.e. aging, children, pregnant teen, HIV/AIDS). 
• Performs nursing assessments on pregnant, parenting, high risk women and their children. 
• Implements care plans to ensure health and safety to specific clients and/or their families.
• Serves as a mandated reporter for child, dependent/elder abuse, and domestic violence.

• Directs  and prioritizes health response during disasters
  
• Provides health care in an emergency shelter during a disaster
  
• Conducts communicable disease surveillance in general shelters opened by American Red Cross
  
• Provides mass vaccinations for disease outbreaks, pandemic influenza preparedness, seasonal immunizations, and other large scale disease prevention events/exercises. 
  

 Population-based Public Health Nursing

• Conducts communicable disease investigations, interventions, and treatment.
  
• Assess data that identifies unmet community health needs, then identifies barriers or new strategies to improve care. 
  
• Serves as the resource or partner to various community collaboratives to address community healthcare needs.
